If you use all 3 attempts without passing, or you do not take the test within the time allowed, IRCC will contact you with next steps. This can include a final notice or an interview with a citizenship official. Check the Government of Canada for the current process, since it can change.
The best way to avoid needing a retake is to walk in prepared. Practising until you consistently score above 75% gives you a comfortable margin.
